Macrolide antibiotics including erythromycin and azithromycin remain important therapeutic options for respiratory tract infections and sexually transmitted infections caused by gram-positive bacteria and atypical organisms. Macrolide resistance mechanisms include ribosomal target modification through methylation (erm genes), enzymatic inactivation through macrolide phosphotransferase or glycosyltransferase production (mph genes), and efflux pump-mediated resistance (mef genes). Ribosomal methylation through ERM enzymes confers high-level resistance to macrolides, lincosamides, and oxazolidinones.
